If your Passport has been lost or stolen, we will help you get started in the right direction. Don’t be overwhelmed. We are here to assist you each step of the way.First, you will need to gather all of the required information (listed below) and then take them to the nearest Acceptance Agent to seal your documents and then send your package to us via Federal Express. You will need to complete two government forms called the DS-11, and the DS-64, as well as the LOA Form (which stands for Letter Of Authorization… it authorizes us to process your passport paperwork). Then download and complete the Universal Passports & Visas Order Form with your payment information and send order form with documents to appropriate office.

After you submit that you have read the disclaimers on the first page, the second
page will have 4 sections. Click “Submit” beneath the option that reads
“Report Lost or Stolen”.

NOTE: The form must be completed online and not handwritten. Then printed. Also,
Do NOT sign the application form until a Passport Acceptance agent
instructs you to do so.

Please attach a second copy of your passport application attached to our Letter of Authorization form. This second copy cannot be given to the passport acceptance office, but must be attached outside the envelope after the information has been sealed.

The Letter Of Authorization simply enables us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and any related documents) when it has been issued by the Passport Agency. Get LOA here.
IMPORTANT: Must print and complete 2 copies of the LOA.

This form must be fully completed and signed. It is needed to prove to the U.S. State Department that you have had your Passport either lost or stolen. It must be completed in detail. Click here to get the form.

This must be one of the following:

  • An original certified copy of U.S. Birth Certificate
  • A previous valid U.S. passport
  • Report of Birth Abroad (Form FS-240)
  • Certification of Birth Abroad (Form DS-1350), or
  • Certificate of Citizenship or Naturalization from USCIS

NOTE: A certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office. The date must be within 1 year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes.

Must be recent, identical and 2×2 inches.

A copy of either your airline ticket(s), flight itinerary or business letter stating travel plans will serve as the required proof regarding your departure date.

This is the government fee. Your payment to us is for expediting of the processing of your paperwork, but the Federal Government will still require its standard fees for Lost or Stolen Passport Replacement.
NOTE: Please indicate your Date of Birth in the memo field of your check.

Payment is $300.00 for 1-2 business days, $250.00 for 3-4 business days, $200.00 for 5-6 business days, $125.00 for 7-10 business days, or $75.00 for 11+ business days. We accept American Express, Visa, MasterCard, Discover or payment by check.
