We will help you get started in the right direction in obtaining a Passport for your Child. Don’t be overwhelmed. We are here to assist you each step of the way.First, you will need to gather all of the required information (listed below) and then take them to the nearest Acceptance Agent to seal your documents and then send your package to us via FederalExpress. You will need to complete two government forms: the DS-11 form, and the DS-3053 form, which must be accompanied by a copy of the non-attending parents’ driver’s license (both front and back). The LOA (Letter Of Authorization) form authorizes us to process your passport paperwork. To qualify, child must be 15 or younger. Then download and complete the Universal Passports & Visas Order Form with your payment  information and send order form with documents to appropriate office.

NOTE: Do NOT sign the application form until a Passport Acceptance Agent instructs you to do so. This form should go inside the envelope from the Acceptance Agent.

Please attach a second copy of your passport application attached to our Letter of Authorization form. This second copy cannot be given to the passport acceptance office, but must be attached outside the envelope after the information has been sealed.

The DS-3053 form must be accompanied by a copy of the non-attending parents’

Note: The form must be completed online and not handwritten. Then printed, and signed.

The Letter Of Authorization simply enables us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and any related documents) when it has been issued by the Passport Agency. Get LOA here.
NOTE: BOTH Parents must sign this for the minor applicant.
ALSO IMPORTANT: You must print and complete 2 copies of the LOA

Must be recent, identical and 2×2 inches.
A copy of either your airline ticket(s), flight itinerary or business letter stating travel plans will serve as the required proof regarding your departure date.
This must be one of the following:

  • An original certified copy of U.S. Birth Certificate
  • A previous valid U.S. passport
  • Report of Birth Abroad (Form FS-240)
  • Certification of Birth Abroad (Form DS-1350), or
  • Certificate of Citizenship or Naturalization from USCIS

NOTE: A certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office. The date must be within 1 year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes.

These are government fees. Your payment to Universal Passports & Visas is for the expediting of your passport processing, but the Federal Government still requires their standard fees for a Child Passport. NOTE: Please indicate the child’s Date of Birth in the memo field of your check.

Download and complete the Universal Passports & Visas Order Form with your payment information and send order form with documents to appropriate office.
Payment is $300.00 for 1-2 business days, $250.00 for 3-4 business days, $200.00 for 5-6 business days, $125.00 for 7-10 business days, or $75.00 for 11+ business days. We accept American Express, Visa, MasterCard, Discover or payment by check.
